&lt;p&gt;Now to your questions:&lt;/p&gt;
&lt;p&gt;1. I did&lt;/p&gt;
&lt;p&gt;2. Yes i tried it on a dk and it worked.&lt;/p&gt;
&lt;p&gt;3. I did, it doesnt work as well, at fails even when all I do is pressing the erase all option.&lt;/p&gt;
&lt;p&gt;4. It is a custom board&lt;/p&gt;
&lt;p&gt;5. I am not sure exactly what HW review means.&lt;/p&gt;

&lt;p&gt;I have a few suggestions for you to check:&lt;/p&gt;
&lt;ol&gt;
&lt;li&gt;Have you tested with other USB cables? Poor-quality cables can sometimes cause issues.&lt;/li&gt;
&lt;li&gt;Have you tried the same setup with other boards to see if the issue persists?&lt;/li&gt;
&lt;li&gt;Could you try using &lt;strong&gt;nRF Connect for Desktop -&amp;gt; Programmer&lt;/strong&gt; to program the device?&lt;/li&gt;
&lt;li&gt;Are you using a custom board or an nRF9160DK?&lt;/li&gt;
&lt;li&gt;If this is a custom board, have you request hw reveiw for your schematic and PCB layout before?&lt;/li&gt;
&lt;/ol&gt;
