Additional information:

I used the PR file, but the issue still occurs. Since everything is the same except for the time, I will omit attaching the logs.

Here are the points that caught my attention:

The bit of active_bitmap is updated in action_set(), but a delayable work continues to run.
In scheduled_action_handle(), since the bit of active_bitmap is not checked, the schedule is executed.

Just an update to let you know that a bug report has been placed and a task to resolve it has been made. I will update you when I have a PR to share and/or with more information. My guess is that a patch might not be targeted before NCS v3.0.0 earliest.
