Hi,

For nRF52840 you can have either "Normal voltage mode" or " High voltage mode". In config 4 which you are referring to, it is high voltage mode. The board(nRF52840) supply is from VDDH and then the VDD_nRF pins which is labeled as VEXT will be the supply from chip to external circuitry. So in this mode VDD pin is acting as supply voltage and can be configured.

In "normal voltage mode", VDDH is connected to VDD pins and acts like normal chip supply pin. You can check config 3 in reference design files for this mode:
