I found the issue. I had to change the I2S Formatting configuration from its default configuration. It was causing all audio to be loud and distorted. Thanks!

The default volume can be changed in the audio driver, by editing OUT_VOLUME_DEFAULT.

An update, although the test tone is loud, the problem appears to be related to the i2s. When I connect the BLE device, the audio is always extremely loud. This audio is transmitted over BLE, not related to generated from the test tone.
