You are sure the disconnect is due to communication error on the chip (reset from APP_ERROR_HANDLER in APP_UART_COMMUNICATION_ERROR case)?

Have you set up UICR and CLOCK register to use 32 MHz crystal, see here. This is only relevant if you start the HFCLK (f.ex. use the radio, it requires crystal). -> There has already been applied.

You are running the ble_app_uart example right? -> yes
