C Revision chip is the issue. You should be atleast on a D revision IC.

Lets begin in the beginning: Versions: What is the software version of the BTLE SDK for Arduino that you are using ? What is the chip marking that you have on the top of the nRF8001 ? What is the OS, IDE and the OS, IDE version that you are using ?

It appears that you are using an older version, so please give us the versions used.

@Hung Bui : This is the Nordic provided example: Example I can't delete the bonding info on the phone as far as I know without having it already connected but did try a different phone with no change. I'm pretty sure this is an ACI issue in the setup of the nordic chip and not an over the air issue.

@David Edwin : Is this the correct function: lib_aci_debug_print(true); ? I tried placing it in the main setup function with no extra debug output.

An SPI trace of the messages on the SPI would be very useful. Since you are using the UNO you can enable debug messages in the lib_aci_init()

@Icero : Please provide exact links to the example(s) you used. Please make sure you erased bond ("Forget this device") on the phone information when you start testing.

A sniffer trace would be useful to see what happened over the air.
