The recommended Eclipse version to use with the GNU ARM Plug-in is 4.5 Mars SR2, see this page.

However, Eclipse Neon.1 appears to works fine,but Neon.2 does not.

I had the same issue, as mikster suggested the problem was the Eclipse Neon IDE. The complete configuration was:

Eclipse Neon.2 Release (4.6.2)
GNU ARM Eclipse plugin 201606210758
GNU Tools for ARM Embedded 4.9 2015q3
Segger JLink Software V612d

It used to work for some time, then stopped running/debugging (hanging on "Launching: Configuring GDB", the console output the same like sajin's) for some reason that I was unable to find. Switching to Eclipse Mars solved the problem.

Which version of Eclipse are you using? Neon or Mars?

Not sure if this is relevant, but I have been struggling with a similar problem. In this document it says:

Neon currently has several problems and is not-recommended: ... on Windows, the OpenOCD debugging hangs while starting

on all platforms, the debugging consoles do not show.
