CFLAGS += -DSOFTDEVICE_PRESENT
CFLAGS += -DBLE_STACK_SUPPORT_REQD
CFLAGS += -DNRF_SD_BLE_API_VERSION=3
CFLAGS += -DS132

If you are using the linker-file found in the same directory, your RAM and FLASH settings might place your application in a region that the bootloader does not recognize. Your options then are:

Flash the softdevice (you can do
this by running
make flash_softdevice

Use a linker file with correct
RAM/FLASH settings (you can use the
file found in
[SDK_ROOT]\examples\peripheral\blinky\pca10040\blank\armgcc\blinky_gcc_nrf52.ld)

I edited it to be GPIO numbers and added the makefile. I didn't specifically flash the softdevice, but I can't tell you with 100% guarantee :P

RE: nRF always landing in the non-existant ResetHandler

Yes, I had to check as the physical pin number depends on the package, it is more convenient to talk about GPIO pin number when talking about software ;) Also some users confuse the physical pin number of the chip with the GPIO pin number used in the application. Which Makefile do you use? Do you have a softdevice flashed to your chip?

RE: nRF always landing in the non-existant ResetHandler

No, of course I mean the physical pin number :) Otherwise I would write GPIO ;)

RE: nRF always landing in the non-existant ResetHandler

When you say your LEDs are on pins 20 and 22, you you mean the pin assignment number on the actual chip?
