Hi, I only uses EK-board. But I think I can answer your second question:

For example, If you have brought EK-board from nordic, They will give you a product key and create a nordic account (register), and go to below link. www.nordicsemi.com/.../nRF51822-Evaluation-Kit

And select the "DOWNLOADS" frame You will see some software (Toolchain, bluetooth protocol stack: S110, S120 etc.) And You can find the user guide to step by step build your development environment.
