Then I think you are for some reason getting into the app_error_fault_handler(). Could you define DEBUG, turn off optimizations and check the error_code, line number and file name you get in app_error_handler()?

Yes through USB, I will test with SDK12 tomorrow. Is it possible for you to test with SDK-14?

How do you connect your PCA10040 to your pc? Through the USB? And can you try with SDK12? I know the uart example works in SDK12 for sure.

Yes, I tested it with both enabled and disabled (default). The problem occurs for both.

Do you have flow control enabled in the uart_init()i.e. .flow_control = APP_UART_FLOW_CONTROL_ENABLED ?
