I used uart app and then added it with the example of the link, so I need to delete BLE_NUS_DEF(m_nus); definition, after that, the problem is disappeared,...thanks.

Sorry but I don't see anything related to CCCD in your code snippet. Can you be more precise in the description? You have Nordic FW which is GAP Peripheral and GATT Server? You create custom GATT Service and Characteristic during the start-up? At that time you also set Notify property of the Characteristic so the stack automatically creates CCCD handle? Do you store the structure with all the handles created by stack (response to characteristic create function call) for later use?
