I guess you are referring to the heart rate monitor sketch code found here, right? From what I can understand, the nrf sdk v. 12.1.0 supports the board header for the arduino primo v2.1 (see the file arduino_primo.h header file under /examples/bsp/). If I were you, I would take a look at the proximity application from sdk 12.1.0, which notifies the nrf toolbox application when the primo is out of range of the phone.

You could take a look at the proximity application code & the nrf toolbox ios code here & see how both sets of code implement the notify functionality.
