I think we need to create a custom GATT service that includes the read, write, and notify/indicate characteristics.

The problem with the proximity application for us, is that on iOS, we can only detect a change in alert level while the app is in the foreground. In our application, we need to be able to read alert changes while the app is in the background on the phone.

Yes, this should be possible. Take a look at the proximity application.

Is there any way to "read" an IAS alert level on iOS?

I believe you are correct, but I will double check with R&D & get back to you soon.

Update: Yes, this is unfortunately correct. The GATT Server has not been implemented on iOS yet.
