As awneil said, this is probably due to the name being cached. Disabling and re-enabling BLE on the phone should help. In any case this is a problem on the phone side and nothing you can fix on the peripheral (except if you change the BLE MAC address so that the phone sees it as a new device, which is probably not what you want to do).

How & Where are you observing this?

Operating Systems like Android and iOS may cache the names ...
