This means that the sequence you entered was not correct. Are you certain that the sequence you entered matches what is shown on the screen on nrfcloud.com?

After repeating some time, i can get to the state where LED4 is solid with LED3 off. From the state diagram, this means connected.

I now go to the nrfcloud account expecting to see the device under LTE devices. Still shows 0 devices. What gives?

yes, i follow the doc and see 3 and 4 blinking before starting seq.

after i input the sequence, led 3 starts blinking (led 4 off) - this means connecting and then it goes back to 3 and 4 blinking. is this an indication of bad connectivity?

Do you see LED 3 and LED 4 blink before you start the input sequence? See the documention for this here.
