Unfortunately I cannot share the schematic, but will try to describe the symptoms and will ask some questions that will hopefully help me understand the problem:

When flashing through Ozone, the softdevice/MBR wasn't being loaded properly onto the device. The board that started working had been flashed with an old project using a different method from Ozone, and this was retained in flash when re-flashing using Ozone. The boards are working now.

Thank you both for your help!
The firmware does load and run on a different custom dev board. Also, of the three new custom boards, one of them did start running and executing the firmware, so it seems unlikely to me to be a problem with the firmware itself. I have no idea why the one board miraculously started working after initially not working, and why the other two continue to not function. I was leaning toward it being something wrong with the crystal, but since you say it is possible to execute code without it, that seems less likely.

Mysterious!

RE: Custom Board Programs, Cannot Debug, Firmware Doesn't Run

1) Nope, 32MHz is not require to flash and executing simple code. It is required if you use the radio

2) Something is not right with your firmware. Either you did not map the memory correctly or your firmware requires the softdevice.

3) No, the 32MHz is by default off.

Have you tested your firmware on a known reference working board such as the DK ?
