Datasheet present 4.8 mA in TX (0 dBm) , is it include MCU current or is it only the radio consumption?

The current in the electrical specifications for the RADIO peripheral is the current consumed by this peripheral only and does not include current consumed by other required resources (clock, etc). You can find current consumption for various scenarios in the Current consumption, and more specifically Radio transmitting/receiving. Note that these numbers are when the CPU is in sleep mode. The Compounded section should show numbers including CPU running, but these are not yet available for the nRF52833. You can check this section for the nRF52840 chip to get an idea of the increase in current from CPU.
