&lt;p&gt;All timers are disabled in SYSTEM OFF sleep, however you can set the nRF to SYSTEM ON sleep where it will consume very little power while it waits for an event. I suggest that you see the Real Time Counter example in the SDK as it implements exactly this. You can find the documentation on the feature &lt;a href="https://infocenter.nordicsemi.com/topic/sdk_nrf5_v16.0.0/rtc_example.html?cp=7_1_4_6_34"&gt;here&lt;/a&gt;.&lt;/p&gt;
