If the supply is correct (3V), and the previously flashed firmware is running, the only suggestions I have is to run recover command in a loop, test with another PC, or test with another debugger (possibly a standalone J-Link debugger).

1. re-checked the voltage- its working corrctly.
2. the last flashed SW is stilll running, but we cannot re-flash the board.
3. we use a regular 10 pin segger between the ISP4520 EVB to the DK (please see attached schematic).
4. the flashing worked a few weeks , but in the last two days , 2 boards were bricked (they cant be flashed).devzone.nordicsemi.com/.../ISP4520_5F00_TB_5F00_Schematic.pdf

Is the chip on the board powered correctly and at the correct voltage level? How are the connections from the Debug out port of the DK connected to the custom board?

Hi,

Can you try to tie the reset pin of the nRF52832 directly to VDD and then run "nrfjprog --recover" command? This has resolved some similar issues for customers before.
