Also I call sd_ble_gap_sec_params_reply() only once as well.
Interestingly enough some of the examples don't use sd_ble_gap_sec_params_reply() at all, for example the hear rate monitor. After I implemented DFU service I noticed that sd_ble_gap_sec_params_reply() in my code called twice so I had to remove one. Another issue was parameter DEVICE_MANAGER_APP_CONTEXT_SIZE which was set to zero and one of the functions returned error. After those fixes I can connect to W8.1. Did you try to run in debug mode and put breakpoint in app_error_handler()? This is how I managed to find my issues
