I am using SDK 11 and SD 4.0.2. I only have phone logs which show connections stopped in the background and only restarted when the app is restarted by the user. I don't think there is anything wrong with nRF advertising. I am trying to find the condition which causes the phone to stop connecting by using nRF test firmware.

What nRF5 SDK and Sxxx softdevice are you using here? Do you have any on-air sniffer log (like nRF Sniffer)?
