&lt;p&gt;The gateway streams two BIS by default and it is up to the headset to choose which BIS to sync to. An implementation of how to switch streams can be found in nrf5340_audio/src/bluetooth/le_audio_bis_headset in the function le_audio_user_defined_button_press(). The source code can also be found on &lt;a href="https://github.com/nrfconnect/sdk-nrf/blob/188a16036ab968bc0f9226ba178ca461548a8c66/applications/nrf5340_audio/src/bluetooth/le_audio_bis_headset.c#L410-L437"&gt;github&lt;/a&gt;.&lt;/p&gt;

&lt;p&gt;Yes, as you can see in the image you shared, you can have two BIS in one BIG from a gateway. The image also shows that one BIG with two BIS is the maximum for a broadcast source (gateway). &lt;/p&gt;
