nrf51-DK fail.txt "TIMEOUT"

I am using my nrf51-DK with mBed for prototyping. I noticed recently, that the DK had stopped broadcasting a BLE service, so I followed instructions to update the firmware in the hope to reset anything that was broken when I noticed the file:

fail.txt

had appeared on the mBed drive, this file just reads:

TIMEOUT

What is wrong and how can I fix this / debug it?

Hi George,

When you hold the reset button an turn on (or plug in) the board, do you have the board appear as BOOTLOADER driver ? If you drag and drop the file nRF51-DK-mbed-IF that you can download from our web. Do you have the same issue ?
