May I suggest that this is posted separately as a question? However, I believe that the problem is that SDK 5.0.0 is made to be used with S110 version 6.0.0, as stated in its release notes. If you try to run it with 5.2.1, you will see problems.
Try upgrading the softdevice to 6.0.0, or alternatively replace the S110 header files from SDK 5.0.0 with the ones from the S110 version 5.2.1 release (which are also the ones included in SDK 4.4.2).

The missing include statement will result in a compile time error, so if you can compile without errors, your problem is not related to this issue.
This post was just to let Nordic know that they should add the include statement to a future version, and it's not fixing a huge issue, it's more like a question of convenience.
