Assume a BLE device already has Bluetooth qualification.

Does FOTA DFU requires new a Bluetooth qualification and additional fees if SoftDevice was updated too?

Is separate Bluetooth qualification required for each SoC/enclosure in a device. For example, if earphones and its charging case have different BLE SoC. Do they require 1, 2, or 3 BLE qualifications and payments to be made?

That depends on what new features the newer softdevice is going to have. If there are differences in the BLE behavior or features, a new qualification is needed.

Depends, If one SoC acts as a proxy or relay for the others and the Bluetooth stack is shared (e.g., TWS with one earphone as the master), a single qualification may cover the entire system else you need separate qualification for each.
