I'm looking at table 23.15.2 in the nRF5 Reference Manual, and it states that TX startup current is 4mA with the DC-DC enabled.
Does that 4mA include the CPU idling, or is is it additional to the CPU current consumption?

The TX startup current comes in addition to other current draws such as CPU, peripherals, etc.
