DFU Errors With MCP

I have recently started getting the following error during a DFU using MCP.

Any idea as to the cause of this? What is the best way to debug this?

I am using the following:

MCP 3.10.0.14 Dongle PCA 10031 V1.1.0 running firmware 0.9.0 or 0.11.0 S132 V 2.0.0 SDK 11.0

I am running with FreeRTOS as well.
