some times  can't update successeful with OTA ,I want to know why？

QDavid: What are you using as a central when performing the DFU? Smartphone( OS version and brand of phone and version of the nrf connect/ toolbox app ) or PC( nRF Connect for desktop) ?

Is the third screenshot after the firmware was uploaded via DFU to the nRF51, i.e. its advertising as DFUTarget after the DFU?

Are you able to start a debug session of the application after DFU? Note: make sure you do not reflash the application when starting the debug session. 

Also can you provide the nrfutil commands you used to generate the DFU image( BLE_OTA_SD)?

Ok, so after you have performed the DFU the 51802 is advertising as DfuTarg, it is not jumping to the application? I am not able to flash the BLE_SDK11_JLink.hex file to my nRF51802 development board as it contains data outside the valid flash area.

Could you share the projects for the application and bootloader you're using so that I can try to reproduce the issue here?

Hi David, I have managed to get the dual_bank_serial_AND_ble_s130 bootloader, found in nRF5_SDK_11.0.0_89a8197__2\examples\dfu\bootloader\pca10028\dual_bank_serial_AND_ble_s130,  to run on my nRF51802 board. 

I did a quick test with a DFU image of the ble_app_hrs app and I did not see any issues. However, you said that this only happens some times. What kind of failure rate are you seeing?
