Why the current will go above 4ma when I add a app_button?

I do my project with the sample BEL_APP_HRS when I add a app_button? The current wii go above 4ma

Did you reset your device after programming before you take current measurements? Also read this.

I have reset it . it is not in debug mode. When I del the gpiote and button the current is 0.2ma . Does the gpiote is reson of the current?

Show the time axis and 100mV = 1mA? In any case, try to measure current consumption in low-power mode (in sd_app_evt_wait(), i.e. don't start advertising), it should be a few uA even with button.
