Can't add lte SIM on nrf cloud

Just got the nrf9160DK and I'm trying to add the Basis LTE SIM to my nrf Cloud account.  However, when I enter the ICCID & PUK, agree to the terms and conditions and click the Verify iBasis SIM button, and error condition pops up that says 'Username and pool information are required.'  Please tell me how to get past this roadblock.
