why s110 only work in keil's debug session

Hardware: NFR51822_QFAAC0

Firmware: S110_7.0_SoftDevice

SDK: NRF518_6.0

Application: ble_app_hrs in NRF51822 S110 Examples

I start a timer to toggle led once per second to show if application is working, but led only blink when I start keil's debug session. Without debug session, my led don't blink, advertising led is on, no Bluetooth device found on my phone.

It could also be that the "Reset and Run" option in your debugger settings is not set. You can either set this, or do a power cycle of your device to jump start it.
