Ways to fall out from Mesh network

Hello,

Could you please help to create a list of reasons (or get a reference to it) when node may fall out from network?

1. Removing network credentials on the node
2. Key Refresh procedure with blacklisted the node
3.  IV Update procedure without the node
4. ... something else?

Hi. 

I can't think of any other reasons other than the reasons you listed above. 
Apart from obvious reasons like the node asserting, being out of range etc. that eventually leads to one of the reasons above. 

I'll take a closer look at the specification and see if I can find other reasons that should force the node out of the network.
