Unable to debug code with softdevice s110 in eclipse kepler

I'm devloping application on nRf51 for that i'm using... SDK : nRF51_SDK_7.2.0_cf547b5 Tool chain : GNU tools arm Embedded [arm-none-eabi version 4.9.3] Dev Board : PCA10028(nRF51 DK) softdevice: s110_nrf51822_7.1.0 eclipse : kepler

when i compile same code. it compiles well and also create bin/hex. also i'm able to run well but in debugging it create problem.in disassembly it shows

Failed to execute MI command:
          -data-disassemble -s 0 -e 56 -- 1
          Error message from debugger back end:
          Cannot access memory at address 0x0

In console

Program received signal SIGTRAP, Trace/breakpoint trap.
0x00000000 in ?? ()

In make file i have added CFLAGS -g and also replaced -O3 with -O0 to add debugging information. Still having same problem. Also tried to add

mon reset 0

in start up tab in debug configuration. After that makefile look like:
