It looks like supporting an external FEM on the LTE RF path of the nRF9160 SiP is not that straight-forward. Any gain in the RX path must be known to the LTE modem, and any gain in TX must be calibrated to meet all requirements for TX. On top of that is regulatory requirements one must follow, so right now I don't think there is any FEM in the market that is suited for the nRF9160 SiP, but this can change in the future.
If this is something that is required by your application I would suggest you contact your Nordic's regional sales manager and give him feedback about this.
