Our BQB certification was refered to SDK16.0 example(ble_app_hrs), and also added HID、DIS and BAS, other services have been certified besides HID.

What test case are you using and are you passing or failing ? Can you share a log from the testing?

This file is a log of the tests and needs to be opened using PTS.

The PTS log does provide a partials picture of the situation, but there are a cupel of things that is not as clear.
From the logs you have provided it looks like the No or cancel was pressed on the pop up action required box and that results in a FAIL
From the HRS.TS doc there are some procedures for the test. 
Are repeating the steps like suggested in the test procedure?
If you are not able to resolve the issue could you provide a sniffer trace of when such a test is preformed. This will provide more information that can be used to follow the chain of events more carefully an might shad some light on the resulting issue.
It might be a configuration issue as well but I am not certain.
