The MPSL FEM driver does indeed take the gain from the FEM into account. The requested TX power is the final output power after the gain stage.
By default the fem TX gain is set to 22dB to support the SKY6112-11, if a FEM with a different gain is used this must be configured with the tx-gain-db property in the FEM devicetree node.
