USB and SWD errors?

Using rev 0218 firmware I am getting an awful lot of device errors with the mbed kit. I am not sure if this is a Nordic or an mbed issue so am asking in both places. Symptoms I observe include

1. Failure to be recognised as an mbed FAT volume on Windows 7x64: Specifically Windows warns that formatting is required before the disk can be used

2. Flashing a new image created using the online compiler fails. A 'fail.txt' file is written to the disk with 'SWD ERROR' as its only content. This on both Windows 7 and Windows 10 hosts.

Has anyone seen similar problems? Both hosts have been used with dozens of mbed devices without problems.

I am having same problem while copying hex file to MBED drive. I am using BMD-200 evaluation kit. Its also connecting & disconnecting automatically
