Must include ICCID or EID to associate a SIM

I'm getting a "Must include ICCID or EID to associate a SIM" message when trying to register a SIM.

And yet the ICCID and PUK have been entered into the form.

What does this message indicate?

I just received a replacement SIM from iBasis and I'm also getting this same message. 

And whats this about deleting last two digits of ICCID ? I've not heard that before, and if I try it it doesn't help

I tried using 3-digit EID number but that didn't work either. Same error

Was nice to get a free replacement SIM, and super easy, but sadly I can't use it
