Do you miss all the information, or only the two items with question marks on them?
It is a bit hard to guess from the French labels what exact information is asked for. Is there a guide explaining the fields of the form? (French descriptions should suffice, if it must be translated then it's better for me to get the original text.)

Duty cycle for BLE is theoretically max at around 60 %, but for testing purposes you can use 100 %.
Occupied Bandwidth (OBW) is 1.05 MHz for 1 Mbps and 2.1 MHz for 2 Mbps. (Normal BLE is 1 Mbps; Bluetooth 5 added 2 Mbps as an option. Most likely you are only using 1 Mbps.)
FCC and CE certification is for the end product (or if you use a module, the module can be certified), but not for the SoC alone. You need to do those certifications yourself (or use a certified module.)
